Papilla gum graft

After: Papilla augmentation surgery was performed to replace the missing gingiva. Orthodontic treatment closed the gap space. Because the teeth were very short, sculpting was performed to create a more natural smile. After the graft I noticed that the papilla ( gum between the teeth) had been removed on one side of two different teeth.


The dentist is acting very defensive and says he has never seen this happen before and just walks out of the room when I try to talk to him about it.

Q: I’ve already been told I’m a good candidate for gum grafting due to the recession I’m experiencing. However, on a recent trip to the general dentist for a standard filling, he seems to have nicked the gum tissue in between two teeth – the papilla I’ve learn it’s called – and now my tongue continues to go right there to notice the tiny gap. Soft tissue reconstruction (also called gum graft or gingival graft ) is a generic name for any of a number of surgical procedures whose combined aim is to cover an area of exposed tooth root surface or dental implant with grafted oral tissue. A toothpick that is pushed between the teeth will actually smash the delicate piece of gum that fits up between the teeth creating the “black triangle” ( dental papilla ), or the dark space between the teeth.


If the tooth on either side has bone loss, even minor, the gum is much less likely to fill in completely leaving a small black triangle. This can be preplanned in advance in many situations. The most predictable and commonly used approach to eliminate the black triangle is to place crowns or veneers on the adjacent teeth.

These have been in place months and although the papilla has filled in between the laterals, the tissues are still flat between the and despite the fact that a periodontist placed the implants doing a graft to “bulk up” the tissue.


Often the loss of papilla is a consequence of periodontal disease because of gingival inflammation, attachment loss and interproximal bone height resorption. Missing papillae can also result from periodontal surgical therapy, as the soft-tissues usually contract during the healing period.
